Candidacy Application Forms
The preparation of a Candidacy Application is to be done entirely online and submitted according to the timeline provided by AOTA accreditation staff. A Word version of the Candidacy Application is available for download below, which programs may wish to use as a working document to assist with compiling the necessary information. Once the institution submits a letter of intent and its eligibility application has been approved, the electronic Candidacy Application will open.
