Candidacy Application Forms
The preparation of a Candidacy Application is to be done entirely online and submitted according to the timeline provided by AOTA accreditation staff. Word versions of the Candidacy Applications are available for download below, which programs may wish to use as a working document to assist with compiling the necessary information. Applicant programs will be provided with login information to the ACOTE Online eAccreditation system once the institution has submitted a Letter of Intent and new program has been set up.
